New Patients Forms & Info

How long will my examination take?

Your initial examination and testing will be comprehensive. We ask that you allow a minimum of 2 to 3 hours for your appointment (possibly longer if extensive testing is required).

Both of your pupils will be dilated in order to perform your retinal eye examination. This makes you very sensitive to bright light. For your safety, it is best to plan for someone to be available to drive you from our office after the examination.  The dilation may last 8-48 hours following the examination (taking longer with darker colored eyes).

New patients are asked to bring the following:

  • Photo identification
  • Insurance cards (Medical insurance, NOT VISION INSURANCE)
  • List of their medications & dosage
  • A driver
  • Completed Patient Info & Consent forms
